Operators are moving fast on AI adoption but keep hitting the same foundational gaps: unmeasured agent costs, fragmented individual use, live security incidents, and weak data governance underneath it all.

Senior operators across technology, manufacturing, medical, and services describe a common pattern: AI is being adopted faster than it is being governed or measured. No one can reliably attach financials to agent token usage, though one firm is piloting a per-hour employee-versus-agent cost model. Staff are adopting AI individually, creating silos and chat-built processes that bypass company standards, which operators frame as "AI for me versus AI for we." Security failures are already real, not theoretical. One coding agent echoed back a secret token in the development environment, and a worm injection forced a two-week code-repository shutdown followed by heavy guardrails. Data-residency guarantees now decide tool choice: one operator cannot use preferred models because the vendor cannot promise data stays in the tenant. Underneath everything sits data governance. Agents surface outdated documents and duplicate file versions that look like hallucinations, and operators call AI a magnifying glass that amplifies poor hygiene. Finally, automation relocates friction rather than removing it. Speeding the front of a process backs up quality control, and executives struggle to see why QC roles still matter, even as agents introduce silent errors during periods of complacency. The through-line for executives is clear. Measurement, alignment, security, and data readiness are prerequisites, not afterthoughts. Firms treating them as foundational will scale AI safely while others accumulate hidden cost and risk.
