Senior operators across technology, manufacturing, and regulated industries described a widening gap between AI adoption and control. Agent use is spreading, but no one could put clear financials behind token costs, and some built session-only memory to limit spend. Security risks were concrete, not theoretical: a developer leaked a live API token into an AI coding tool and the agent echoed it back, while another firm shut down its code repositories for two weeks after a worm injection. Data residency uncertainty is already blocking tool choices, with one operator refusing outside models inside Copilot because data could leave the company tenant. Uncoordinated individual adoption is fragmenting processes, which operators framed as AI for me versus AI for we. Many failures traced back to weak data foundations rather than model quality, as AI surfaced outdated files and unsecured documents that looked like hallucinations; one operator called AI a magnifying glass for poor hygiene. A developer warned that reliable AI QA breeds complacency, then quietly introduces errors months later. The common thread: operators are building, automating, and shipping faster than they are measuring, governing, and securing. Executives should treat cost attribution, data cleanup, sensitivity labeling, and recurring output audits as preconditions for scaling AI, not later cleanup. The operators who paused to fix foundations reported fewer surprises; those who raced ahead described mounting friction, silent errors, and downstream quality control bottlenecks.
