The AI Value Gap: Why Spending Runs Ahead of Measurement and Control
Insights from senior operators across industry and healthcare on AI measurement, cost, security, governance, and workforce adoption.

Operators are moving fast on AI while admitting they cannot yet measure its cost, secure its data, or delegate its ethics.
Senior operators across industry and healthcare converged on a gap between AI enthusiasm and AI control. The clearest signal was measurement: token consumption runs unchecked, managers demand AI use but never ask how it is used, and cost-cutting is mistaken for real value. Security drew equal concern. Staff paste contracts and customer data into external tools, not realizing they sit outside company control, and breaches stay an afterthought until they happen. Operators noted firms sign short-term deals and wait rather than commit, using rapid technology change as a reason to defer real investment. On governance, the group was firm that ethics cannot be programmed into AI, leaving the human in the loop as the accountable layer. A healthcare-adjacent example raised wearable recording of patient interactions, where one-party consent laws let devices capture protected information beyond a company's control. Adoption itself splits by generation, with newer staff embracing tools and veterans staying skeptical, forcing leaders to balance encouragement and restraint. The practical advice was consistent: identify the highest-ROI processes because AI is expensive, find a champion, assign a budget, align plans to measurable returns, and train people before asking them to use the tools. The throughline is discipline. Operators are not short on AI ideas. They are short on baselines, guardrails, and ways to prove that spending turns into value rather than exposure.
