Operators are moving fast on AI adoption but keep hitting the same foundational gaps: unmanaged data leakage, opaque agent costs, siloed use, and weak data governance that surfaces as failure.

Senior operators across technology, manufacturing, medical, and services described an AI landscape where enthusiasm is outpacing controls. The most common worry is data leakage: employees paste proprietary information into public models to work faster, with no central oversight. Concrete incidents were shared, including a coding agent that echoed back a secret token and a worm injection that shut down a code repository for two weeks. New guardrails then created their own friction, and operators are still searching for balance. Agent economics remain opaque. Several cannot attach reliable financials to token usage, and are testing cost-per-task frameworks comparing agents to human labor. Adoption is fragmenting into silos as staff build private workflows off company standards, a pattern one operator called 'AI for me versus AI for we.' Data governance emerged as the shared foundation. Agents pulling outdated or unsecured files produce outputs that look like hallucinations but reflect poor file hygiene; one operator described AI as a magnifying glass on bad management. Finally, automating the front of a process was found to push friction downstream to quality control, whose value is hard to justify to executives. The signal for leaders is clear: the barriers to AI value are not model quality but data readiness, cost visibility, coordinated adoption, and security discipline. Firms that fix these foundations first will scale AI safely while others accumulate hidden debt.
