Senior operators across varied industries agreed on one control for AI: keep a trained human in the loop. But they admitted that control is often hollow. Many reviewers simply hit approve without reading what AI produced. The fix is specific coaching on how to evaluate outputs, not just naming a reviewer. Beyond oversight, advisors raised knowledge management. Employees build valuable prompts and skills, then take them when they leave, and few firms capture that know-how. This is a quiet loss of institutional capability. A more sensitive concern surfaced too. Employees are forming relationships with AI and turning to it for mental health support. That personal data is often owned by the vendor or the employer, and almost no one has policy for it. Compliance maturity varied sharply. Some firms enforce strict AI rules while others operate as a wild west. Despite that gap, every operator agreed that legal compliance matters and that a trained human must review AI work to protect the company, its employees, and its students. The takeaway for executives is that the popular safeguards are real but underbuilt. Human oversight without reviewer training gives false assurance. Knowledge walks out the door uncaptured. Sensitive employee data enters tools with unclear ownership. Leaders should treat these as live risks now, set baseline oversight rules, train reviewers, capture AI knowledge, and write policy before adoption outpaces control.
