Senior operators across pharmaceuticals, banking, aerospace, healthcare, and technology converged on one tension: demand for AI agents is outpacing the foundations needed to run them safely. Teams build agents faster than platforms can support, creating sprawl with inconsistent permissions and no shared observability. Several warned of technology-first adoption, where organizations build agents to justify using AI rather than defining real requirements and circuit breakers first. Measurement has replaced budgeting as the central challenge. Operators now track metrics like time saved per engineer, but efficiency gains create new problems. Workers fear saved time signals replacement, making trust an unsolved issue. Operators agreed on what matters: a named human must own each agent, with continuous drift monitoring and clear accountability. More than technical skills, they prioritized critical thinking, analytical thinking, and forward planning. The message for executives is clear. Production AI demands foundations, honest metrics, trust narratives, and human accountability before scale.
