Operators are past piloting AI and are now hitting concrete costs, security incidents, and data-hygiene failures that outpace their ability to measure and govern them.

Senior operators across technology, manufacturing, and regulated industries described a shift from AI enthusiasm to operational reckoning. Agent costs are the sharpest worry: one operator cannot attach real financials to token usage, and costs creep as agents multiply. Security incidents are already happening, not hypothetical. A developer pasted a live credential into an AI coding tool that echoed it back, and another operator shut a code repository for two weeks after a worm was injected. Data residency doubts are limiting tool choice, with one Microsoft-centric leader banning a rival model because the vendor could not guarantee data stays in the company tenant. Data hygiene emerged as foundational. AI surfaces outdated and duplicate files that look like hallucinations, and disorganized data gets amplified into unreliable output. Automation also relocates friction rather than removing it, pushing bottlenecks onto quality control teams whose value is now hard to defend to executives. Finally, AI tools introduce silent errors during periods of complacency, eroding the vigilance that reliable output creates. The common thread is that operators are moving faster than their measurement and governance can keep up. Executives should instrument agent costs before scaling, clean and label data before deployment, protect quality-control capacity, and build fast incident-reporting and audit routines. The operators who treat data readiness and cost measurement as prerequisites, not afterthoughts, are the ones avoiding the worst surprises.
