Operators are moving fast on AI but keep hitting the same unsolved foundations: unmeasured agent costs, fragmented usage, data leakage through new agent paths, and messy data that quietly corrupts output.

Senior operators across industry describe a widening gap between AI ambition and operational control. Agent adoption is outrunning measurement: nobody could put real financials behind token usage, and one team removed session memory purely to control spend. Individual AI use is fragmenting standards, described as AI for me versus AI for we, raising both silo and intellectual property risks. Security concerns were concrete, not theoretical. A coding agent echoed back a sensitive token, and one firm shut GitHub for two weeks after a worm injection. An IT operator said his company blocks third-party models in Copilot because tenant data containment cannot be guaranteed. Data hygiene emerged as the quiet root cause of unreliable output. Outdated file versions make agents produce wrong answers that look like hallucinations, and one operator called AI a magnifying glass for poor management. Automation is also relocating work rather than removing it. Front-end automation backs up quality control, and leaders struggle to justify QC roles to executives expecting pure savings. Finally, operators warned that agents introduce silent errors during complacency and that sycophantic, agreeable AI can pull users into groupthink. The common thread is that foundations, measurement, governance, security, and data readiness, remain unsolved even as deployment accelerates. Executives should slow down long enough to fix the foundation, because speed without it simply amplifies existing weaknesses.
