Operators are finding that AI's real risks are not model quality but ungoverned adoption: opaque agent costs, leaking secrets, silos, dirty data, and shifting bottlenecks.
Senior IT and technical operators described a consistent gap between AI enthusiasm and the controls needed to run it safely. Agent costs are the sharpest surprise: several admitted they cannot forecast token spend, and one now deliberately disables session memory to control it. Security incidents are concrete, not theoretical. One developer watched a coding agent echo back a secret token he had just pasted, forcing a self-report and log cleanup. Adoption is fragmenting. Staff use AI independently, creating silos and processes that ignore company standards, a pattern one operator called "AI for me versus AI for we." Data hygiene emerged as the true foundation. Agents pulling outdated file versions produce what looks like hallucination, and one operator described AI as a magnifying glass that amplifies existing disorganization. Automation also moves problems rather than removing them: front-end efficiency backs up quality control, and agents introduce silent errors after long stretches of clean output. Finally, data residency uncertainty is limiting model choice, with one operator ruling out a preferred model because tenant containment could not be guaranteed. The common thread is governance lagging usage. Executives should treat cost metering, secret scanning, data cleanup, and QC capacity as prerequisites for scaling AI, not afterthoughts. The operators who set shared standards and clean their data first are positioned to capture team-level gains their rivals will miss.
