Others echoed the pattern of speed outrunning quality. A research reviewer receives grant proposals and peer reviews with raw AI prompts still pasted in, unread. A print trainer sees clients ship low-resolution AI images that fail to print. A sales engineer is micromanaged on AI usage even when manual work is faster and more accurate. A data VP is squeezed from both sides: users connect warehouses straight to LLMs without caring if results are valid, forcing her to build governed analytics agents while managing new tech debt like cognitive drift. A nurses union has banned AI entirely as a worker-protection stance. Multiple operators reported hallucinations, including identical prompts returning conflicting answers and a model fabricating a future trend before admitting it made it up. The common thread: organizations are deploying and mandating AI before validating it, cutting staff before proving savings, and democratizing data access faster than they can govern it. Executives should measure outcomes rather than usage, validate performance before reducing headcount, require human review of all AI output, and build governance layers instead of bans. The confidence gap between how AI is sold and how it performs is where the real risk sits.
