Senior operators across technology, communications, manufacturing, and regulated industries described a common pattern: enthusiasm for AI is outrunning the foundations needed to run it safely. Individual, uncontrolled use is creating silos and workflows that ignore company standards. Agents surface outdated files and expose data in unsecured locations, producing errors that look like hallucinations but reflect weak data governance. One developer accidentally leaked a live API token to an AI coding tool, which echoed it back and triggered a self-reported incident. An IT leader now blocks any model that cannot guarantee data stays inside the company tenant, showing how data residency alone narrows vendor choice. Cost is equally unresolved. Operators admit they cannot put financials behind agent token usage, and are experimenting with per-hour employee-versus-agent frameworks as prices shift rapidly. Automation itself relocates rather than removes bottlenecks: one operator found that automating the front of a process backed up quality control, while executives undervalued the QC role.
