Senior operators across manufacturing, security, legal, and higher education describe a shift from AI excitement to disciplined scrutiny. Pilots are stalling because organizations cannot inventory what AI is running or stop duplicated, siloed builds. Cost has become the central tension. One manufacturer noted that AI ROI is capped while the cost of achieving it shifts every six months, so contracts must lock price over five years. The same leader refuses to rent intellect through SaaS, warning that leaving a vendor risks losing all the knowledge staff taught the tool. Productivity gains are real, over 100 percent in one case, but framed as a long game absorbed through growth rather than layoffs. Several operators questioned agent economics, arguing skilled humans still beat agents on cost for coding. The sharpest concern was human-in-the-loop control. Operators admitted reviewers often just click approve, and that reviewers with incentives to deny claims create conflicts of interest. This turns oversight into false assurance while quietly shifting accountability. Proper training, honest incentives, and clear purpose for the reviewer emerged as the fix. Underneath runs a change management gap: top-down mandates fail without bottom-up reasons to adopt. Executives should treat governance, cost modeling, knowledge ownership, and reviewer training as connected priorities, not separate projects. The through-line is a demand for honest measurement of what AI actually costs and delivers.
